*Trips available to Wakulla and Leon Counties, but individuals may request transportation outside these areas in special circumstances *Non-emergency transportation for anyone of any age in need: (1) Medicaid recipients * (call your insurance company 1st); (2) seniors and other persons who qualify as "Transportation Disadvantaged" due to disability, low-income, age or other extenuating circumstances and anyone without transportation *Transportation arrangements to health care appointments for Medicaid clients is made by the *HMO to which the client is assigned; the client does NOT make the arrangements with a provider directly *Persons who do not know if they are in a Medicaid HMO should call AHCA, Medicaid Area 2B Field Office: 800-248-2243 or 850-412-4002 to find out *CTD qualified riders should call the local provider directly 24 hours in advance to make arrangements for transportation; CTD application needed for lower cost *Persons not in a Medicaid HMO can still use the local provider, but will have to pay out of pocket, unless a sponsor covers them *May have an escort if necessary, that rides free; parent(s) needing to take additional children to an appointment must pay a fare for the Children and provide car seats and safety equipment *If a car seat is needed for an underage child, adult must supply it

*Permanent supportive housing for chronically homeless individuals with disabilities which are specifically set up so persons with various disabilities can be accommodated including wheelchair accessible affordable efficiency apartments and on-site supportive services staff *Sliding scale fee rent, though all residents must have some income *Access is gained through a coordinated entry process; social workers/case managers from agencies who work with homeless persons (such as the Kearney Center or Ability 1st) meet bi-monthly and rate each person according to need; The person(s) with highest rated need are given assistance in completing paperwork and documentation to become eligible when there is an opening
