Taylor County Recovery Center, Inc.

*DCF licensed out-patient substance abuse treatment program for ages 13 and over including adults; individual and group counseling, substance abuse education, treatment planning and review, discharge planning, relapse prevention, probation reporting *Other DCF licensed classes open to anyone in the community: parenting classes, dealing with trauma, co-dependency, employability skills training, money management, HIV/AIDS education, domestic violence, batterer's intervention, anger management, conflict resolution, Drivers' License Assistance Classes for Driving while License Suspended/Revoked (DWLS/R) *Can be self-referred or ordered by the court system for any services offered *DCF ACCESS Community Partner Site: anyone may make an appointment to go there to apply for Medicaid, Food Stamps/SNAP and/or TANF

Thanksgiving Tallahassee

*Annual free Thanksgiving Meal at the American Legion Post 13 at 229 Lake Ella Drive, 32303, 11am - 1pm, on Thanksgiving Day for people who are alone or have no one with whom to share the holiday including homeless persons, older adults, families in need, college students, members of law enforcement, others who work on Thanksgiving Day or are away from families; no one will be turned away *Guests receive a traditional Thanksgiving meal, entertainment (a band), and free long distance phone service to speak to loved ones *Round trip free Star Metro bus transportation will be provided from and back to: The Kearney Center, Hope Community, Westgate, Home Front, Hayden Arms and Tallahassee Veteran's Village, Balkan Road; additional stops can be added for groups of 15 or more; wheelchair and bicycle transport available *Volunteers needed for food prep, cooking, serving, set-up, clean-up, transport of food, transport of individuals, etc.

TMH, Transition Center

*Facility to provide follow-up care to certain patients only after they get services from Tallahassee Memorial Hospital or any TMH health facility (including the emergency room), either as an inpatient or outpatient *Includes patients without a primary care provider, without health insurance, who are not able to obtain a follow-up appointment with a physician within seven days of discharge, who have been hospitalized three or more times over the last 12 months *Services may include a physical exam, help with filling prescriptions, arranging physical therapy and other types of rehabilitation services, connecting patients with other agencies as needed to help them continue their recovery

Trans Lifeline

*Hotline staffed by transgender volunteers for transgender people experiencing a crisis, including people struggling with gender identity offering information on transition related health care or support groups in the area and able to assist with covering the costs for non-cisgender people to update the following documents: passport, driver’s license, state ID, court order name change, immigration documents, and tribal IDs when funding is available *Will call emergency services only with expressed consent *Family & Friends Line provides peer support for friends, partners, family members and professionals supporting trans loved ones and community members

UM, Comprehensive AIDS Program

*Confidential and anonymous HIV testing *Works to medically treat HIV/AIDS patients and to partner with county/federal government agencies to establish a means for patients with HIV/AIDS to get treatment regardless of their ability to pay *HIV symposium, held annually, combines opportunities for all level of HIV/AIDS researchers with presentations, poster displays, and panels with community partners, promotion of education and mentorship programs, and partnering with community organizations, the CFAR aims to participate in the initiative to End the HIV Epidemic by implementing strategies to Diagnose, Treat, Prevent, and Respond *Promotes scientific interest groups: Vaccine & Immunology, Cure/Reservoirs, Health Equity (includes Prevention, Substance Abuse), HIV and Women, Ending the HIV Epidemic (EHE) and AIDS Related Malignancies *Facilitates patients ability to get treatment from any UM medical department *Primary care, psychosocial counseling, and other specialty care services to include case management

*Infectious Diseases Clinical Program provides specialists to provide board-certified consultants for infants, children and adolescents with problems associated with HIV/AIDS *Comprehensive primary care HIV/AIDS program for children infected at birth or at risk for HIV *Testing, counseling, evaluation services, recommendations and referrals at Mailman Center for Child Development *CMS offers nursing case management services *Ryan White Title IV Miami Family Care Program assists women, children and youth and their families in accessing HIV services *Adolescents are offered specialized care coordination to meet unique needs of this age group *Dept of OB/GYN provides care for HIV-infected women coordinating health, mental health and referrals to counseling and referrals
